ABSTRACT

RESUMEN Objetivo: Identificar las características clínicas y los criterios quirúrgicos en el síndrome de Brown. Métodos: Se realizó un estudio descriptivo retrospectivo a 15 pacientes con el diagnóstico de síndrome de Brown, quienes acudieron a la Consulta de Visión Binocular del Hospital Infantil Sur "Dr. Antonio María Béguez César". Se incluyeron los niños con edades entre 2 a 7 años, con el diagnóstico definido de síndrome de Brown; se realizó exploración motora y sensorial completa y se evaluó el ojo afectado, el tipo de síndrome de Brown que tenía el paciente, las características clínicas y los criterios quirúrgicos tenidos en cuenta para resolver una conducta adecuada. Resultados: En la muestra estudiada el 66,5 por ciento de los pacientes tuvo afectado el ojo derecho con un síndrome de Brown leve; el 73 por ciento estuvo en ortotropía y el 73,3 por ciento no fue quirúrgico. Conclusión: Los pacientes con síndrome de Brown leve que permanecen en ortotropía, sin tortícolis y sin depresión en aducción, no requieren intervención quirúrgica(AU)


ABSTRACT Objective: Identify the clinical characteristics of and surgical criteria for Brown syndrome. Methods: A retrospective descriptive study was conducted of 15 patients diagnosed with Brown syndrome attending the binocular vision service at Dr. Antonio María Béguez César South University Children's Hospital. The sample included children aged 2-7 years with a definite diagnosis of Brown syndrome. Complete motor and sensory exploration was done, and evaluation was performed of the affected eye, Brown syndrome type, clinical characteristics and surgical criteria considered to select the appropriate management. Results: 66.5 percent of the study sample had their right eye affected by mild Brown syndrome, 73 percent were orthotropic, and 73.3 percent were not surgical cases. Conclusion: Patients with mild Brown syndrome who remained orthotropic, without torticollis or adduction depression, did not require surgical intervention(AU)

ABSTRACT

Resumo Objetivo: Identificar a prevalência de alterações visuais em crianças de cinco anos em escolas públicas de Curitiba-PR. Métodos: As escolas foram selecionadas aleatoriamente dentro do município de Curitiba. As crianças com cinco anos completos em 2017 foram avaliadas com a tabela de Snellen, através de distância mínima correta para nitidez de imagem e teste de Hirschberg. Os pais responderam um questionário sobre uso de telas, sintomas oculares e histórico familiar da criança. Os resultados das avaliações foram analisados estatisticamente considerando nível de significância p≤0,05. Resultados: Em uma população de 459 crianças triadas, 219 (47,7%) pertenciam ao sexo feminino e 240 (52,3%) masculino, sendo que do total, 100 foram encaminhadas para avaliação oftalmológica especializada. A partir da triagem observou-se a prevalência de miopia de 10,7%, hipermetropia de 17,6% e estrabismo de 0,9%. Houve relação entre genitores com miopia e filhos míopes (p<0,05). Dentre as queixas oftalmológicas predominaram cefaleia (30,4%) e franzir de testa (10%). Conclusão: A prevalência de alterações visuais encontrada foi de 21,8%. A relação entre distúrbios visuais e o histórico familiar se mostrou estatisticamente significativa. Entretanto, apenas o tempo médio em frente à televisão apresentou influência, dentre os hábitos de vida, sobre as alterações da AV (p=0,028). Queixas oftalmológicas apesar de frequentes, não apresentaram correlação expressiva com a diminuição da acuidade visual.


Abstract Objective: To identify the incidence of visual impairment in 5-year-old children in public schools from Curitiba-PR. Methods: A selection of schools has been choosen randomly from Curitiba. The children, with completed 5 years at end of 2017 have been evaluated using Snellen table, trough minimum distance for image sharpness and Hirschberg test. Parents answered a questionnaire about the use of screens, ocular symptoms and family history of the child. Significance levels were defines as begin p≤0,05. Results: The results have shown that four hundred fifty-nine children were screened. Two hundred nineteen are female (47,7%) and two hundred fourty (52,3%), male. From all screened patients, one hundred were refered to specialized oftalmic evaluation. After trial completing, has been attained a prevalence value of 10,7% for myopia, 17,6% of hyperopia and 0,9% of strabismus. Strong correlation between parents and children has been undiscovered (p<0,05). From listed oftalmic complaints, headache (30,4%) and frown (10%) where most prevalent. Conclusion: It has been attained that the prevalence of visual acuity is 21,8%. The relation between visual acuity alteration and familiar history has been shown to be significant related. On the other side, the average time in front of television has been shown the only habit that has correlation with visual acuity reduction (p=0,028). Vision complaints, although very frequent, doesn't translate into increased probability of visual acuity alteration.

ABSTRACT

Se realizó un estudio longitudinal, descriptivo y retrospectivo en el Hospital Oftalmológico Amistad Argelia-Cuba, el Oued, Argelia, en el período comprendido entre junio de 2015 a mayo de 2017, con el objetivo de caracterizar el comportamiento quirúrgico en pacientes con estrabismo atendidos en dicho hospital. El universo estuvo constituido por 80 pacientes intervenidos quirúrgicamente, coincidiendo con la muestra. Las variables estudiadas fueron: edad, sexo, diagnóstico preoperatorio, dioptrías prismáticas preoperatorias, proceder quirúrgico, ojo operado y complicaciones transoperatorias oculares. De las 139 cirugías practicadas hubo predominio del sexo femenino. El 38.8 por ciento de los casos presentó esotropía comitante adquirida. El proceder quirúrgico más realizado fue la recesión de ambos rectos horizontales, predominando la cirugía bilateral sobre la unilateral(AU)


A longitudinal, descriptive, retrospective study was carried out in the Ophthalmological Amistad Hospital Algeria-Cuba, El Oued, Algeria, in the period from June 2015 to May 2017, with the aim of characterizing the surgical behavior in patients with strabismus attended in said hospital. The universe consisted of 80 patients surgically treated, coinciding with the sample. The variables studied were: age, sex, preoperative diagnosis, preoperative prismatic diopters, surgical procedure, operated eye and transoperative ocular complications. Of the 139 surgeries performed / operated eyes, 52.5 percent of the patients corresponded to the female sex. 38.8 percent of the cases presented comitant acquired esotropia. The most performed surgical procedure was the recession of both horizontal rectums, in 84.9 percent, bilateral surgery prevailing over the unilateral one(AU)

ABSTRACT

Abstract Purpose: to determine the prevalence of different types of strabismus submitted to a surgical procedure in a public hospital in the Brazilian Federal District. Methods: Retrospective cross-sectional review of medical records of patients who underwent strabismus surgery performed by the same surgeon from 2004 to 2014. Incomplete records were excluded from the study. Variables included sex, age, type of strabismus, type of surgery, and re-surgeries. Strabismus was classified into three main types (esotropia, exotropia, and pure vertical deviation), and their subtypes. The type of surgery was determined by the number of muscles operated on (up to two, or more than two), the involvement of oblique and vertical rectus muscles, and muscle displacement. Results: 563 patients were reviewed and 531 were included in the study. The average age was 12.7 years, and females accounted for 54.4% of total patients. Esotropia was the most frequent type of strabismus (74%), followed by exotropia (23.7%) and pure vertical deviation (2.3%). The most frequent subtype of esotropia was infantile (49.1%), with 16.1% of these surgeries performed before the age of 2. Constant exotropia was the most frequent subtype of exotropia (27.8%). The prevalence of esotropia decreased with age, whereas that of exotropia increased. 52.3% of surgeries involved more than two muscles. An association between different types of deviation was found in 58.9% of patients. In cases of paralytic strabismus, the sixth nerve was the most affected (46%). Re-surgeries accounted for 10.7% of total surgeries. Conclusion s: Esotropias were the most common types of strabismus, especially infantile esotropia. Most cases were associated with other types of deviation, and needed more complex surgeries, involving more than two muscles. This study expects to contribute to the planning of health measures that may effectively improve the assistance provided to the population.


Resumo Objetivo: Determinar a prevalência dos diferentes tipos de estrabismo submetidos a intervenção cirúrgica em um hospital público de Brasília, Brasil. Métodos: Estudo retrospectivo de corte transversal de prontuários médicos dos pacientes submetidos a cirurgias de estrabismo por um mesmo cirurgião, de 2004 e 2014. Foram excluídos prontuários incompletos. Variáveis pesquisadas incluíram sexo, idade, classificação do estrabismo, tipo da cirurgia e reoperações. Os estrabismos foram classificados em três tipos principais (esotropia, exotropia e desvio vertical puro) e seus subtipos. Avaliação do tipo da cirurgia considerou o número de músculos operados (intervenção em até 2 ou em mais músculos), o envolvimento de músculos oblíquos, retos verticais e transposição muscular. Resultados: Foram avaliadas 563 pacientes e 531 foram incluídas na análise. A média de idade foi 12,7 anos e o sexo feminino respondeu por 54,4%. A esotropia foi o tipo de estrabismo mais frequente (74%), seguido pela exotropia (23,7%) e desvio vertical puro (2,3%). O subtipo de esotropia mais frequente foi o infantil (49,1%), com 16,1% destas cirurgias realizadas antes dos 2 anos de vida. A exotropia constante foi o subtipo mais observado (27,8%) de exotropia. A proporção de esotropia diminuiu com a idade, enquanto a de exotropia aumentou. 52,3% das cirurgias envolveram mais de dois músculos. Associação de diferentes tipos de desvio foi observada em 58,9% dos pacientes. Dentre os estrabismos paralíticos, o VI nervo foi o mais acometido (46%). Reoperações corresponderam a 10,7% do total. Conclusão: As esotropias foram os estrabismos cirúrgicos mais comuns, especialmente a esotropia infantil. A maioria dos casos estava associada a outras formas de desvio e necessitaram de cirurgias mais complexas, envolvendo mais de 2 músculos. Espera-se que dados do estudo possam contribuir no planejamento de ações de saúde efetivas para melhorar a assistência à população.

RESUMO Objetivo: Investigar a associação da hipermetropia com ambliopia, estrabismo, anisometropia e astigmatismo. Métodos: A hiperopia foi classificada em Grupo 1: maior ou igual a +5.00D; Grupo 2: maior que +3.25D e menor que +5.00D, com diferença de equivalente esférico maior ou igual a 0.50D; Grupo 3: maior que +3.25D e menor que +5.00D, com diferença de equivalente esférico menor que 0.50D e Grupo 4: com equivalente esférico maior e igual a +2.00D. O Grupo controle pertencente ao equivalente esférico menor que +2.00D. Resultados: A presença de hipermetropia maior e igual a SE+2.00D foi significativamente associada à maior proporção de crianças com ambliopia (27,2 vs. 14,8%, OR = 2,150, p<0,001) e estrabismo (70,8 vs. 39,3%, OR = 3,758, p<0,0001. A presença de hipermetropia também foi significativamente associada à maior proporção de anisometropia nos grupos com hipermetropia maior e igual a SE+2.00 (29,1 vs. 9,9%, OR = 3,708, p<0,0001) e astigmatismo (24 vs. 9,9%, OR = 2,859 p<0,0001). Conclusão: A presença e magnitude da hipermetropia entre crianças foram associadas à maior proporção de erros refrativos e visuais, como estrabismo, ambliopia, astigmatismo e anisometropia.


RESUMO Objective: Evaluate ocular trauma cases related to falling in elderly patients e compare the prevalence and severity of the cases. Methods: A series of cases was made with 52 patients aging 60 or more within the period of 36 months presenting ocular trauma related to falling, whereas the prevalence between the gender, the need for hospitalization or surgery and subsequent visual deficit were evaluated, as well as the severity of the cases. Results: Thirty-three (63.5%) of 52 patients were from the female gender, over which 30.3% had need for surgery and 18.2% developed visual deficit and 19 (36.5%) were from the male gender where 42.1% needed surgery and 26.3% developed visual deficit. Conclusions: The study has shown a higher prevalence of cases in the female gender, although the severity was higher in the male gender.

RESUMO Objetivo: Identificar e analisar fatores de risco associados ao estrabismo, dados oftalmológicos e antecedentes pessoais e familiares. Métodos: Estudo prospectivo, descritivo e analítico realizado com 240 pacientes que consultaram pela primeira vez nos ambulatórios de estrabismo. Foram avaliadas a frequência e associação entre estrabismo, dados de exames oftalmológicos e antecedentes pessoais e familiares para identificação dos fatores de risco. Resultados: Observou-se maior prevalência de estrabismo em mulheres (56,96%) em relação aos homens (43,02%). A faixa etária mais atendida no ambulatório de estrabismo foi composta de crianças até 15 anos de idade (83,54%). Encontrou-se mais esotropia (55,27%) que exotropia (42,20%) e parcela menor de estrabismo vertical isolado (2,53%). A prematuridade foi associada ao estrabismo convergente (p=0,023) e não ao divergente (p=0,086). O diabetes mellitus materno foi associado à esotropia (p=0,024) e exotropia (p=0,036) no filho, mas a hipertensão arterial materna, uso de medicamentos para hipertensão e diabetes durante a gravidez e drogas ilícitas não foram estatisticamente significativos. Pacientes com paralisia cerebral tiveram mais associação com exotropia (p=0,008) que esotropia (p=0,019). Malformações congênitas, consanguinidade entre os pais e traumas oculares não apresentaram associação estatisticamente significativa para o estrabismo. A maioria dos pacientes com estrabismo apresentou boa acuidade visual corrigida e fundoscopia normal. Na análise de proporções a hipermetropia foi mais frequente na esotropia (68,7%). Conclusão: Os fatores de riscos internos e externos podem interferir na formação e desenvolvimento do olho na fase de plasticidade ocular, com prejuízo da binocularidade visual, podendo resultar em estrabismo. Há divergências com relação aos fatores de risco realmente associados e mais estudos são necessários para a identificação.


ABSTRACT Objective: To identify and analyze risk factors associated with strabismus, the eye data and personal and family history. Methods: A prospective, descriptive and analytical study, with 240 patients who consulted for the first time in strabismus clinics. We evaluated the frequency and association between strabismus and data eye examination and personal and family history to identify risk factors. Results: There was a higher prevalence of strabismus in women (56.96%) compared to men (43.02%). The age group most satisfied in strabismus clinic are children up to 15 years of age (83.54%). Met more esotropia (55.27%) than exotropia (42.20%) and lower portion of isolated vertical strabismus (2.53%). Preterm birth was associated with convergent strabismus (p = 0.023) but not the diverging (p = 0.086). Maternal diabetes mellitus was associated with esotropia (p = 0.024) and exotropia (p = 0.036) on the child, but the mother’s high blood pressure, medications for hypertension and diabetes during pregnancy and illicit drugs was not statistically significant. Patients with cerebral palsy association with exotropes had more (p = 0.008) which esotropia (p = 0.019). Congenital malformations, consanguinity between parents and ocular trauma showed no statistically significant association for strabismus. Most patients with strabismus showed good visual acuity, normal fundus. In the analysis of proportions farsightedness was more common in esotropia (68.7%). Conclusions: Internal and external risk factors can interfere in the formation and development of the eye in eye plasticity phase with impaired visual binocularity and may result in strabismus. There are differences with respect to risk factors associated truly and more studies are needed to identify.

ABSTRACT

PURPOSE: Single umbilical artery (SUA) is the most common malformation of the umbilical cord. However, there have been no studies on the ocular findings in SUA, except for one case report. This study aimed to investigate the ocular findings in children with SUA. METHODS: Fourteen children (eight boys and six girls) with SUA were evaluated retrospectively. All children underwent a complete ophthalmologic examination. RESULTS: The prevalence of abnormal ocular findings in children was up to 42.9%. Refractive errors are detected in four eyes (14.3%): myopia > or =-1.50 diopters (D) in one eye (3.6%) and hyperopia > or =+2.00 D in three eyes (10.7%). Epiblepharon was found in three children (21.4%), and strabismus was detected in one child (7.1%). CONCLUSIONS: Approximately half of the children with SUA showed abnormal ocular findings, therefore, our case series highlight the need for a comprehensive ocular examination and larger prospective research studies in young patients with SUA.

ABSTRACT

Objetivo: Reportar hallazgos oftalmológicos en niños prematuros examinados antes del año de edad gestacional corregida (EGC). Pacientes y Método: Revisión retrospectiva de fichas clínicas de una serie de prematuros ([peso nacimiento (PN) < 1 500 g y/o < 32 sem edad gestacional (EG)] nacidos en CAS entre 2006-2009, examinados antes del año de EGC. Se consignó EG, PN, antecedente de retinopatía del prematuro (ROP), EGC al examen, refracción, anisometropia y estrabismo. Resultados: De 149 prematuros nacidos en el período, 100 tuvieron examen oftalmológico antes del año de edad (67,14 por ciento); el promedio de EG y PN fue 29,3 (rango 2336) semanas y 1217 (343-2190) g, respectivamente. El 29 por ciento presentó ROP, requiriendo tratamiento el 4 por ciento. La EGC promedio al examen oftalmológico fue 6,4 (3-11,5) meses. El equivalente esférico fue +1,34D (-1,75 a +5,75), presentando anisometropia >1D el 2,2 por ciento(95 por ciento IC, -0,82-5,26). El 3,3 por ciento (95 por ciento IC, -0,38-7,04) presentaba indicación de lentes según recomendaciones internacionales y el 4 por ciento (95 por ciento IC, 0,16-7,84) presentó estrabismo. Conclusiones: La baja prevalencia de factores de riesgo para ambliopia detectada en esta cohorte comparada con la reportada para prematuros con ROP severa podria explicarse por la baja prevalencia de ésta última en la presente serie.


Objective: To report ocular abnormalities in premature infants examined before one year of Corrected Gestational Age (CGA). Patients and Methods: Retrospective review of medical records of a number of premature infants ([birth weight (BW) < 1 500 g and/or < 32 week gestational age (GA)] born between 2006 and 2009 and examined before one year of CGA. GA, BW, retinopathy of prematurity (RQP), CGA examination, refraction, anisometropia and strabismus information were recorded. results: Out of the 149 premature infants born during the period, 100 had eye exam before one year of age (67.14 percent), the mean GA and BW was 29.3 (range 23-36) weeks and 1217 (343-2190) g, respectively. 29 percent had ROP, 4 percent required treatment. The mean CGA at the time of ophthalmologic examination was 6.4 (3 to 11.5) months. The spherical equivalent was +1.34 D (-1.75 to +5.75), 2.2 percent presented anisometropia > 1D (95 percent CI, -0.82 to 5.26). 3.3 percent (95 percent CI, -0.38 to 7.04) were in need of glasses according to international recommendations and 4 percent (95 percent CI, 0.16 to 7.84) had strabismus. Conclusions: The low prevalence of risk factors for amblyopia detected in this population compared with that reported for premature infants with severe RQP could be explained by the low prevalence of the latter in this study.

ABSTRACT

OBJETIVOS: Identificar a frequência de ocorrência de desvios oculares e as características dos portadores em uma amostra populacional. MÉTODOS: Estudo transversal, observacional e probabilístico, entre os anos de 2004 e 2005, envolvendo 11 cidades da região centro-oeste do estado de São Paulo. Foram examinados 10.994 indivíduos, sendo utilizada para este estudo uma subamostra desta população, identificada pelo diagnóstico de estrabismo. A população foi abordada por uma equipe treinada e padronizada para os procedimentos da pesquisa. Os dados foram analisados estatisticamente por meio de análise descritiva, frequência de ocorrência, análise de contingência e testes de associação (p<0,05). RESULTADOS: A frequência de ocorrência de estrabismo na população estudada foi de 1,4% (148 portadores de estrabismo), sem diferença entre sexos. Portadores de esodesvios (ET) eram 46,3%, 38,2% casos de exodesvio (XT) e 15,4% de desvios verticais associados a horizontais ou síndromes. A análise de contingência mostrou que 3 indivíduos (2,3%) estrábicos apresentavam cegueira e 7 (5,43%) apresentavam baixa visão em um dos olhos. Tanto a ET, quanto a XT estiveram presentes em indivíduos com graus variáveis de miopia (até -5,75 para XT e -2,50 para ET) e de hipermetropia (até +9,00 para XT e +8,00 para ET). A associação entre estrabismo e o equivalente esférico obtido na refração estática não mostrou diferença significativa (p>0,05). CONCLUSÃO: A frequência de ocorrência de estrabismo em uma amostra populacional foi de 1,4%, sem diferença entre sexos ou tipo de desvio ocular. A presença de cegueira e de baixa visão associadas aos desvios oculares reforçam a necessidade de tratamento precoce.


PURPOSES: To identify the frequency of occurrence of ocular deviation and the carriers characteristics in a population sample. METHODS: Observational cross-sectional study, evolving a probabilistic sampling, between the years 2004 and 2005, including 11 cities of the São Paulo state. 10,994 individuals were examined. A sub-sample of this population identified as strabismus clinical diagnosis was the subject of the present study. The population was addressed by a trained team and standardized to the research procedures. The results were submitted to statistical tests through descriptive analysis, frequency of occurrence evaluation, contingency analysis and association tests (P<0.05). RESULTS: Strabismus frequency of occurrence in the population studied was on average 1.4% (148 strabismus carriers), with no difference between gender. Depending on the type of strabismus detected, 46.3% presented ET, 38.2% cases of XT and 15.4% of deviations were associated with vertical or horizontal syndromes. Contingency analysis showed three ocular deviation carriers (2.3%) with blindness (both eyes affected) and seven (5.4%) had low vision in only one eye. According to the association of the ocular deviation and refractive error, ET and also XT can be present in individuals with varying degrees of myopia (up to -5.75 for XT and -2.50 for ET) or hyperopia (up to +9.00 for XT and +8.00 for ET). The association between strabismus and static spherical equivalent showed no significant difference between the types of eye deviation (P>0.05). CONCLUSION: The frequency of occurrence of strabismus in the population studied was 1,4%, with no difference between gender or ocular deviation type. The presence of blindness and low vision associated to ocular deviation reinforce the need for early treatment.

ABSTRACT

To determine the prevalence of visual impairments among children in primary schools. This cross-sectional study was conducted between from November 1st to December 31st 2006. Visual acuity was determined with the Snellen's E chart. Visual impairment was defined as visual acuity of d"20/40 in the better eye, as amblyopia if the difference in visual acuity between both eyes was 0.2d"dioptry, and as strabismus if the cover test was positive. Of the study group [n=1606], 53.7% [n=864] were boys and 46.3% [n=742] girls. The mean age of the participants was 10.52 +/- 2.28 [range 6-17]. The girls had a higher frequency of a presenting visual impairment than boys [2.4% vs. 1.0%], [p<0.05]. Twenty seven [1.7%] children presented with vision of d"20/40 in the better eye. The prevalence of presenting visual acuity for d"20/40 was significantly higher in children with glasses compared to the group of children without glasses [p<0.05]. The prevalence of amblyopia was 5.0%, whereas that of strabismus was 1.7%. The number of students with visual impairments was high, and many students were unaware of visual impairments and their risk factors

ABSTRACT

OBJETIVO: A Fundação Altino Ventura realizou o projeto "Enxergando através das mãos", com o objetivo de investigar a situação atual da múltipla deficiência no estado de Pernambuco, verificar a demanda reprimida nesta área e analisar relevância para implantação de um centro especializado de referência para o diagnóstico e tratamento desses pacientes. MÉTODOS: Foi avaliado o perfil de um grupo de pacientes atendidos em cinco serviços de referência do grande Recife. Os casos foram avaliados quanto a ocorrências perinatais, inclusão social e dificuldades de acesso aos serviços especializados. Promoveu-se o atendimento oftalmológico especializado e o tratamento indicado para cada caso. O projeto foi desenvolvido de forma prospectiva, seguindo protocolo e organograma previamente elaborados. Os atendimentos foram realizados em seis dias não consecutivos, durante os quais todos os casos passavam pelas etapas de entrevista dos genitores e exame oftalmológico completo. Durante o período de espera entre entrevista e exame oftalmológico os pacientes realizavam atividades lúdicas com artistas plásticas. Contou-se com equipe multidisciplinar de 31 profissionais. RESULTADOS: Foram atendidos 309 pacientes. Estavam incluídos na sociedade 83,1 por cento dos pacientes, apenas 45,4 por cento freqüentavam escola (normal ou especial). A dificuldade de acesso aos serviços na área de oftalmologia foi relatada por 51,1 por cento dos casos. Foram identificadas 274 crianças com múltiplas deficiências, destas, 154 (56,2 por cento) nunca haviam sido examinadas pelo oftalmologista e 33 por cento tinham baixa de visão (inferior à 20/60 na tabela de Snellen). Os diagnósticos oftalmológicos mais freqüentes foram: erro refracional, estrabismo, ambliopia, alterações do nervo óptico (glaucoma e atrofia), e catarata. Foram prescritos e doados 84 óculos. PURPOSE: The Project "Vision through the hands" was performed by the Altino Ventura Foundation, with the purpose of investigating the present child situation with multiple disabilities in the state of Pernambuco, Brazil. METHODS: Birth and social conditions, social problems, and difficulty in accessing the specialized services were evaluated, aiming to collect relevant data with the goal of posterior implantation of a specialized diagnosis and treatment center. Ophthalmologic care and proper treatment were offered. RESULTS: Three hundred and nine children from five reference institutions in disabilities were attended, on six non consecutive days. All cases underwent ophthalmologic examinations and parent interviews. During the waiting time between the ophthalmologic visits and parent interviews the patients performed activities with artists. A multidisciplinary team of 31 professionals was available. According to parent information 83.1 percent of the patients were considered included in the society and only 45.4 percent attended school (normal or special). The difficulty to get access to ophthalmologic service was reported by 51.1 percent. Two hundred and seventy children with multiple disabilities were indentified, 154 (56.2 percent) of whom underwent an ophthalmologic examination for the first time, and 33.0 percent had low visual acuity (inferior to 20/60 Snellen). The most frequent diagnoses were: refraction error, strabismus, ambliopia, alterations of optic nerve (glaucoma and atrophy), and cataract. Eighty-four glasses were prescribed. CONCLUSION: This project was pioneer in the region, allowing to obtain the information about important problems regarding multiple disabilities, and making possible ophthalmologic care and specialized treatment to the children.

ABSTRACT

Strabismus or eye deviation is one of the important fields in pediatric ophthalmology and is the most common cause of amblyopia. Amblyopia is one of the leading of vision loss and monocular blindness and because it develops in the early years, it affects an individual for life. The earlier amblyopia is identified, the more successfully it is manged. In this analytical descriptive study, 107 patients, who referred to department of ophthalmology, Emam Reza hospital, from 2000-2003, were selected and a questionnaire was filled for each patient according to the medical records. Data were analyzed using SPSS 10 statistical software. 42.1% of our patients were male and 52% of them was female. Mean age of the patients was 13.22 years. Distribution curve of ages has two pick in 2-3 years and 14-19 years. According to the data, 86.0% of patients referred from Central Khorasan and only 0.9% of South Khorasan. 47.7% of patients were unaducated and 60.7% of them were esotropia. Amblyopia was seemed in 20.6% of the patients. The results of this study confirm the high prevalence of amblyopia among the strabismus patients. As attendance at follow up is often poor, pediatricians should be aware of the association to encourage opportunistic assessment and ophthalmological surveillance of this population. It is advisable to screen and perform optometrical and opthalmological examination, in the age that children are predisposed for amblyopia to prevent it

ABSTRACT

Introducción: la fibrosis y contractura muscular determina la aparición de un estrabismo restrictivo. Objetivo: describir aspectos clínicos-radiológicos y resultados quirúrgicos en una serie de pacientes. Materiales y métodos: estudio retrospectivo de 22 pacientes operados en 2 centros clínicos entre 1990 y 2005 con tabulación de datos clínicos, mediciones en 9 posiciones, ducciones, resultados quirúrgicos y complicaciones. Además análisis doble-ciego clínico radiológico. Resultados: mayor prevalencia en mujeres (88 por ciento), (x) edad 57 años, síntoma inicial diplopia (100 por ciento). Compromiso muscular RI (72 por ciento), RM (63 por ciento), RS (22 por ciento), RL (9 por ciento). Grupos más frecuentes: RM bilateral (18.2 por ciento) y RI unilateral (16 por ciento). Resultado postoperatorio excelente (sin diplopia) 75 por ciento, satisfactorio (sin diplopia con prismas) 23 por ciento y malo (diplopia permanente) 2 por ciento. Correlación clínico-radiológica exacta (45 por ciento) y del principal músculo afectado (100 por ciento. Complicaciones: 33 por ciento ptosis palpebral inferior y 9.5 por ciento neuropatía óptica compresiva. Conclusión: el manejo estrabológico adecuado en orbitopatía de graves otorga en un alto porcentaje una mejoría de sus síntomas y mejor calidad de vida.


Introduction: fibrosis and muscular contracture results in restrictive strabismus. Objective: describe clinical-radiological features and surgery results in a group of patients. Methodology: retrospective study of 22 surgical patients treated in 2 ophthalmology institutes between 1990 and 2005. Ciinical data, 9 positions and duction study, surgical results and finally incidence of complications were all assesed. Also a double-blind clinical-radiological correlation was performed. Results: female were more affected (88 percent), mean age 57 years, initial sympton was diplopia en every case. Muscle involvement IR (72 percent), MR (63 percent), SR (22 percent), LR (9 percent). Most frequently muscle associations: both MRs (18.2 percent) and unilateral IR (16 percent). Postoperative results: excellent (diplopia free) 75 percent, satisfactory (diplopia corrected with prisms) 23 percent, bad (persistent diplopia) 2 percent. Exact clinical-radiological correlation (45 percent), mayor affected muscles correlation (100 percent). Complications: inferior palpebral ptosis (33 percent) and comprenssive optic neuropathy. Conclusions: adequate strabismus management in graves orbithopathy gives the patient symptomatic relief and better life quality in a high percentage of cases.

ABSTRACT

Objetivo: Describir los estrabismo asociados al Síndrome de Parinaud (SP). Métodos: Se estudiaron pacientes con SP de 1998 a 2004 asociados a otros estrabismos. Resultados: Total 18 pacientes, 6 mujeres y 12 hombres. Edad: 28 años DE 22 (10; 1C 95 por ciento). Lesión al III nervio craneal en 15 pacientes (79 por ciento), II nervio: 8 pacientes (44.4 por ciento), VII nervio: 3 pacientes, hemiparesia: 3, lesión a IV nervio 2, parálisis bilateral de III nervio 2, nistagmus 2, Síndrome de foville 2, y síndrome de uno y medio 1 paciente. Causas: Accidente vascular cerebral 4, uno de ellos asociado a infarto agudo de miocardio, traumatismo cráneo-encefálico 3, tumor cerebral 6, neurocisticercosis 2. Se obtuvo ortoposición con toxina botulínica en 1.72 +/- 1.1 (0.65 IC 95) y cirugía en 9, mejoría espontánea 1. Conclusiones: Identificar el diagnóstico topográfico y pronóstico de la lesión cerebral asociada a SP es muy importante en el control y vigilancia del paciente así como para ofrecer una rehabilitación mas dirigida.


Objective: To describe the causes of Parinaud syndrome (PS) with diplopia to determine a topographic diagnosis and prognosis. Method: We studied patients with SP from January 1980 to January 2004; causes and treatment. We identified all strabismus associated to PS. Results: We included 18 patients with diplopia, 6 female and 12 male. Age: 28 years old, SD 22 (10; CI 95 percent). Related strabismus: III nerve palsy in 15 patients (79 percent), optical nerve damage in 8 (44.4 percent), VII nerve palsy in 3, IV nerve palsy 2, bilateral III nerve palsy 2, nystagmus 2, Foville syndrome 2, One and half syndrome 1. Causes: Tumor 6, brain stroke 4, cranial trauma 3, neurocisticercosis 2. Ortoposition with botulinum toxin treatment was obtained with 1.72 +/-1.1 (0.65; IC 95 percent) inyections. Surgery was done in 9 patients, only one patient had spontaneous resolution of diplopia and SP. Conclusions: To identify the topographic diagnosis and prognosis of brain diseases in PS is very important to complete the study of strabismus. In this way we can know more about of damage extension and we can identify recurrences quickly to offer a better control.

ABSTRACT

Propósito: El objetivo del siguiente trabajo fue el de determinar la prevalencia y los distintos tipos de trastornos de la motilidad ocular en una población escolar de la sierra ecuatoriana. Material y métodos: Se examinaron 6143 niños de entre 4 y 16 años de edad de dos áreas geográficas étnicamente similares de la Sierra del Ecuador. De la población examinada, 2775 fueron de sexo femenino y 3368 de sexo masculino. A cada niño se le efectuó un examen oftalmológico que comprendía: inspección, estudio de la motilidad ocular, reflejos pupilares, estereopsis, agudeza visual sin corrección y con corrección en aquellos niños portadores de lentes, examen de ojo externo, autorrefractometria y videorrefractometria. Se efectuó autorrefractometria con cicloplejia y examen de fondo de ojo a todos los niños con niveles de agudeza visual menor a 0.8 en alguno de sus ojos, a todos los niños entre 4 y 6 años y en forma randomizada al 10 por ciento del total de los niños. El examen de la motilidad ocular consistió en evaluación de la excursión ocular hacia las 9 posiciones de la mirada, Cover test para lejos y cerca con y sin corrección, test de Hirschberg y determinación de la presencia o no de un tortícolis. Resultados: De los 6143 niños examinados 371 (6 por ciento) presentaba algún trastorno en su motilidad ocular. Estrabismos latentes (forias) fueron encontrados en 253 niños (4.11 por ciento). De ellos, 249 casos tenían exoforias y solo 4 presentaron endoforias. Esotropias pudieron observarse en 22 niños (0.35 por ciento). Exotropias se presentaron en 17 de los niños examinados (0.27 por ciento). Se presentaron 8 casos con anisotropía en V e hiperfunción de oblicuos inferiores y 55 pacientes con anisotropía en Y sin disfunción de músculos oblicuos. Dos pacientes presentaron desviación vertical disociada. Presentaron nistagmus sin estrabismo 5 niños y parálisis del IV nervio pudieron observarse en 2 casos


Purpose: The purpose of this paper is to determine the prevalence and different types of ocular motility disorders in a population of school children of the Highlands of Ecuador. Patient and Methods: 6143 children with ages between 4 and 16 where examined in two etnically similar areas of the Highland region of Ecuador. 2775 where females and 3368 males. Every child underwent an opthalmological examination that included: Inspection, evaluation of ocular motility, pupilar reflexes, stereopsis, visual acuity without correction and with correction in all the children wearing glasses, external examination, autorrefractometry and videorrefractometry. Autorrefractometry with cycloplegia and retinal examination were performed in all children with vision levels less than 0.8 in a least one eye, to ocular motility evaluation consisted in assement of the eyes excursion in all 9 positions, of gaze Cover test for distance and near, Hirschber test and determining the presence of an anomalous head posture. Results: Of the 6143 children evaluated, 371 (6 percent) presented with some type of ocular motility disturbance. Of theses, 253 (4.11 percent) where latent strabismus. 249 suffered from exophorias and only 4 had esophoses. There where 8 children with V pattern and overaction of inferior obliques and 55 with Y pattern and no disfuction of oblique muscle.

ABSTRACT

Purpose: To estimate the prevalence of strabismus in Natal, Brazil, among elementary and high school students of the public and private educational systems, in addition to detecting etiological factors. Methods:1024 students were randomly selected and submitted to a questionnaire and a complete ophthalmologic examination, by professors and resident physicians in Ophthalmology at the Federal University of Rio Grande do Norte. Results: Of 1024 students, 1015 were examined; 29 were found to have strabismus (2.9 percent), 20 of whom had manifest exotropia (2 percent), 2 had intermittent exotropia (0.2 percent), 6 had esotropia (0.6 percent) and 1 had V anisotropies (0.1 percent). Conclusions: The strabismus prevalence of the student population of Natal falls within the range of the worldwide population. There was ocular lesion in only one student (retinochoroiditis scar on the posterior pole in both eyes) related to strabismus.

OBJECTIVE: To assess the prevalence of visual impairment and ocular abnormalities among schoolchildren in Chiang Mai. DESIGN: A community-based survey. SUBJECTS AND METHOD: The vision screening project was conducted from June 2000 to March 2002. Students in grade I in the Chiang Mai municipal area were examined for visual acuity (VA), color vision, ocular alignment, anterior segment and fundus. Subjective refraction was done in students with subnormal vision (VA 20/30 or less). Referral to the hospital for further evaluation and treatment was made for students with strabismus, amblyopia and other ocular abnormalities. RESULTS: A total of 3,431 and 3,467 students were enrolled in 2000 and 2001, respectively. The prevalence of normal vision (VA 20/20), VA 20/30 or better in at least one eye and 20/40 or less in at least one eye were similar in both years (87%, 5.7%, 7.3% and 85%, 6.4%, 8.7%, respectively). There was no statistically significant difference in visual acuity among boys and girls in either year (p = 0.6 and p = 0.2). Prevalence of abnormal color vision was 4.2%. Other causes of visual impairment in both years included strabismus (1.5% and 6.2%), amblyopia (1.1% and 1.4%) and some congenital abnormalities. Most cases of amblyopia were due to uncorrected refractive errors. CONCLUSION: The authors found that over 10% of school-aged children had subnormal vision. The important causes of visual deterioration came from refractive errors, strabismus and amblyopia. The authors concluded that vision screening is a cost-effective way of reducing visual morbidity from preventable visual impairment, which is a tragedy that cannot be ignored.

ABSTRACT

An ophthalmic survey of 310 school going children in the age group of 4-12 years was done. 41 children (13.2%) were found to have a vision of less than 6/6 in one or both eyes. Myopia of 0.5 D or more and hyperopia of +2 D or more or astigmatism of 1 D or more in one or both eyes were seen in 74 children (22.9%). There was a tendency for a decrease in hyperopia and an increase in myopia with advancing age. Astigmatic error of 1 D or less was seen in 12 eyes (1.93%) and anisometropia of more than 1 D was present in 11 cases (3.5%). Hypertropia was more in female children whereas myopia had a nearly equal distribution in both sexes. There was heterophoria and manifest strabismus which showed a gradual increase with advancing age. This study was compared to few other studies undertaken in the past.
